About Us

The Hammersmith & Fulham Health and Care Partnership, also referred to as a place or borough based partnership was formed in 2016 (when it was known as the H&F Integrated Care Partnership).

 

Following the Health and Care Act 2022 the landscape of the NHS changed. The Kings Fund have produced a short video explaining these changes and how the NHS in England now works.

Partnership meetings

Below you can find details of key H&F Health and Care Partnership meetings.   

 

Board

  • Quarterly meeting chaired by the H&F Health and Care Partnership Co-chair and made up of Executive level leaders of our partner organisations

  • Oversees the work of the partnership and is responsible for decision making and ratification of decisions made elsewhere

  • The workstreams and boards report into the H&F Health and Care Partnership Board

Executive Leadership Group

  • Fortnightly meeting chaired by H&F Health and Care Partnership Director

  • Core members are leaders of partner organisations with additional attendees by invitation only

  • Meeting purpose is to set strategic vision for the partnership, collaborative decision making and problem solving

​

Operational Delivery Group

  • Fortnightly meeting chaired by H&F Health and Care Partnership Director

  • Brings together Operational Managers and strategic leaders from partner organisations and lay representatives

  • Meeting purpose is information sharing, celebrating success and good practice, collective problem solving and a space for collaboration

​

Workstream Meetings

  • The new workstream meetings for 23/24, Access to health and care services, Integrated Neighbourhood Teams and Tackling health inequalities take place every 6-8 weeks 

  • The existing workstream meetings, Diabetes, Frailty and Mental Health also take place every 6-8 weeks

  • There are also a number of workstream subgroups which meet regularly
