H&F Patient Reference Group Elects New Chair

Even though I’m the Chair, I’m not the most important person of group, it’s the residents who attend the meeting who are the most important people. For me, I will strive to build a working relationship with the H&F Health and Care Partnership, to ensure we have true co-production and to raise concerns from the Patient Reference Group.
The next meeting is taking place 15th June 2023, 2.30-4.30pm in person at the Irish Cultural Centre. If you are interested in joining the group, please contact Val on 07768801932 or email nhsnwl.hfhcp@nhs.net

Dying Matters Week Event
Hammersmith & Fulham residents are invited to join us for our Dying Matters awareness event, Thursday 11th May 2023, 10am -1.00pm at Parkview Centre for Health and Wellbeing (56 Bloemfontein Road, White City, London, W12 7FG).
Information Stalls in the Atrium:
Join representatives from the NHS, Adult Social Care, Hospices, Carers Network, End of Life Doula UK and local support networks. Find out more about end of life care in our community; and discuss how and where we provide care for patients and their families, when they need it most.
We will also have a dedicated space for people to come together for our interactive workshop sessions:
10:30 - 11.30am - Power of Planning Ahead
We will explore individual choices when it comes to future care, known as Anticipatory/Advance Care Planning and the power that comes with making a plan. With opportunities to share your thoughts and information about the tools you can use to make a plan.
11:30am to 1.00pm - Compassionate Conversations
An opportunity to have one to one conversation with our expert hosts or come together in small groups to talk about death, dying and grief.
We will be providing refreshments at the event. Feel free to pop in and take a look at the wealth of information and support available. Or book a ticket in advance on Eventbrite.
